In order to maintain compliance with the Administrative Mandate on Required COVID-19 Vaccination, new and rehired employees in all classifications (regular, term, on-call, temporary, and non-credit earning teachers) are now required to demonstrate compliance with the Vaccine Mandate on or before the start date of employment (eff. Oct 12, 2021).

Compliance documentation will incldue either (1) documentation of full vaccination using a vaccine authorized by FDA License or under an FDA Emergency Use Authorization (EUA), or (2) appropriate documentation seeking an available exemption. If exemptions are requested, they must be fully approved on or before the start date of employment.

An A&S Staffing Request form must be submitted to the Dean’s Office (via email to with Cc to Rob and Shirley Rey) for all Regular, Term, Temporary, and On-call positions funded by non-I&G money. This request must be completed for all Staff changes to FTE before an ePAF is processed, but it does not have to be completed for Faculty and/or Student (Grad/Undergrad) employment transactions. Please forward all A&S Staffing Requests with supporting employment documentation to with Cc to Shirley Rey and Rob Armijo.

Staff hiring and personnel requests funded by I&G money require an EVP Request form, to be forwarded to Rob and Shirley Rey together with a CAS Staffing Request and updated Org Chart.

NOTE: Approvals are a 2-step process. The first email notice from HR will document the approval of your action (posting or hire). The second approval notice will let you know that the action has been completed (either the job was posted or the offer letter information will be sent to the candidate.

New form for converting temporary Staff to Regular Staff

Criminal Conviction Certification (CCC) and a Staff Justification of Hire Memo. (Required eff. June 2017)  

      Within the "Staff Justification of Hire Memo," indicate “yes” and attach the appropriate education documents/transcripts to the hire. Or, if you viewed the Education documents/transcripts within the selected employee’s HR file, indicate “no”  and then type as documentation: Education documents/transcripts are at HR within the selected candidate’s HR file.
