Effective Thursday, April 27, 2017, the New Version of the PTI Letter of Understanding (LOU)  must be used. This can be found on the OFAS website list of forms.

If changes need to be made to the original LOU after the PTI has signed it, the department is required to submit a new PTI LOU with new signatures on it.

• Let the PTI know that this is stated in the LOU that they signed. Refer to the last paragraph of the PTI LOU (just above the signature line) so that they understand why they are signing another LOU.
• When you submit a the new PTI LOU, please write “Revised on MM/DD/YYYY” on the upper right hand corner of the new PTI LOU.
